Any fare will do. Even a booking through a travel agent (thank goodness, since I have to use my corporate travel agent for work bookings, and our policy is "strictly economy bookings.")

No. Award availability is a good sign loads are low, but I've been upgraded on segments with no award availability before. It may be that award inventory is capped per flight, but some dates/times/city pairs just have low paid demand up front. There'll sometimes be enough unsold inventory after that upgrades will still clear.

Correct. Collective wisdom is that if there's award availability in business, upgrades will reliably clear - but there's no guarantee per se.
