This is a HUACA situation. I have had the "Entree Failed to Save" error and the agents have been able to assist with meal selection. As others have noted, make sure they are selecting the pre-select option you want and not a "special meal."
This worked; the agent suggested the issue was it was a complimentary upgrade while the underlying ticket was still at a Main fare class. She added the PDML but also suggested at the check in point it would be able to save it too. She was correct, at the 24 hour point I still had the limited options and could save it from the app.
