At the 30-day mark, I filled out and submitted the online upgrade request form and it seemed to process correctly. The next day, I called to confirm everything was in order. The agent said that she did not see an upgrade request. She then accessed my HawaiianMiles account, verified I had an available upgrade certificate, attached it to my desired segment on my reservation and then confirmed that I was on the upgrade .
Was your AS # in the reservation? I looked for the online form after logging in and never could locate it but it sounds like calling is the way to go regardless.
