I assume the 4 OTPs are for guests, no? You get full UC access.

The Gold benefit is illusory though - as Gold you have access to Star Alliance lounges anyway.

Here's the text of the benefit - I don't see ambiguity:

Primary Cardmembers receive a value of at least $750, including additional guest privileges with their United Club Membership. Primary Cardmembers and their eligible travel companions, including 1 adult guest and dependent children, will enjoy access to 45+ United Club locations worldwide.

Unlock All Access membership as a Cardmember after reaching Premier Gold status or by spending $50,000 on purchases each calendar year with your United Club Card.In addition to your membership benefits, you will have: Total of 2 adult guests or 1 adult guest plus your dependent children under the age of 18 will have access to all United Club locations.* Access to participating Star Alliance – member affiliated lounges for you and your eligible travel companions.
