Based on my experience, I flew with them on J, and my birthday is 2 days after prior my departure schedule. And they did put on happy birthday mark on my boarding pass.

They dont give me a cake, (because they give the surprise gift right after delivered the dessert for me) but they give me written happy birthday on a post card, also miniature of A350-1000 with hbd mark on it.
