In the UK, you really don't need to ask to see a manager to get a service charge removed. I also don't like them, so I merely tell the serving staff that: "I'd like to leave a tip that definitely ends up with you, but I can't do so unless you remove the service charge". Not failed yet. There's no need to over-complicate things or produce a wordy document that most people will neither know anything about nor have the power to influence the policy. The manager you're aiming it at, assuming he or she even has a choice in the matter, quite likely won't even get to see it.
