I have the opposite experience with TK - my calls, at least in J, we're always answered pretty much immediately and I got what I wanted within 2-3 minutes (perhaps except during the main meal service on longhaul, which is a mess). The reason why I'm mentioning it is that this is one of those cultural differences you talked about above. I strongly prefer this style of service, where no one bothers me, but when I need something it's available. On the other hand, you have a bunch of Americans complaining online how they went hours without a drink because no one offered them any - and using the call button didn't occur to them, or they didn't feel like they can do it. You can't really adjust the service to satisfy both sides as our needs are completely opposite.
That's because Americans have been scolded enough times domestically on the point that the call button is not to be used for service requests....
The use of the call bell is an excellent example of cultural differences.
