What goes wrong at some airports is that flights arriving in the non schengen area are not separated into clean/unclean and everyone arriving from non schengen countries is considered unclean.
And that produces the mess. In LHR all incoming flights except the domistic ones are considered as unclean. Also the jetbridge (except some gates) are not designed to handle clean/unclean countries. When they would do it, transfers in LHR would so smooth. And any transfer between Terminals has to be consideres as clean (except public transport).
