Most likely an e-ticket has been issued and you just do not have the e-ticket number. What makes you think the e-ticket has not been issued?
It used to be you could find the e-ticket number on the Avianca App. Have you tried looking there? If all else fails you can always call in and ask LifeMiles for it.
