People refer to an Account Manager, similar to those for Business and Corporate accounts, not Telephone Customer Care Reps. They are assigned based on spend and other factors. The criteria are disclosed.

Amex does have certain designations for its card members based on various criteria, such as spending, high-profile individuals, high-value clients, etc. So if they call the call center and identify themselves (the last four digits of their SSN and the last five digits of their account), they get directly routed to a more experienced Customer Care Rep or group.
