Today I cancelled and re-booked (better rate) and the new reservation now correctly shows as Amex Fine Hotesl and Resorts. Will update once I see the FHR credit post / 5x points.

I contacted Amex through their website chat and, while the rep seemed extremely lost (it's currently 10pm PT, I may have been connected to an offshore rep), they were able to give me the extra 4x points (plus the initial 1x already posted, for an accurate total of 5x) which they said would post in 0-24 hours.
Rep also mentioned they put in a request for the $200 credit to be applied and that it could take up to 5 business days to be reflected. Will update once the points + credit post (or after 5 business days if they don't). Big kudos to FT – what a great place where I can find the one person in the world somehow in almost the same situation.

FYI. scroll up. extensive discussions about this exact issue. AMEX knows and ive been on the phone w/ their team to investigate. cases were opened.
