- is it accessible to all hotel guests free of charge or is that a fee per visit?

- Confirming that this is the typical japanese onsen experience where it's nude only and male/female separate
In respect to the final point - yes - typical japanese. Locker rooms are separate (obviously), as are sauna, steam room and spa (hot tub). Nude only (in both rule and practice) w/ showers readily available.
We enjoyed the spa as part of massages so I can't recall if there was/would have been a fee.
