Just updating my experience in case it's helpful to others. After submitting the form and restaurant receipt mentioned above via email on Thursday, this morning I got an email saying they've mailed me a check for the full amount. Hopefully that arrives soon. All in all, a much less painful process than I was expecting (although it would be helpful if they could just make things easier to navigate on the website).
