Yes the Partner Airline claim form is not working.

Just telephone in your details, or use
ba.com/complaints
making very sure you select the Avios + Executive Club option, even though you may not be bothered by the Avios. The reason being it will be handled by IAG Loyalty rather than Customer Relations.

Telephoning in does assuredly work, but you do need to leave 28 days before following up.
