Indeed many rental car companies do that.

But it appears Sixt does not, or so they claim. I have booked on sixt.ca, sixt.com, sixt.de, never an issue. Logging on using my account.

It appears that on web sites geared toward North America, they do not include damage insurance, which makes since since most folks have credit card insurance. However the same is not true in Europe and then they include basic insurance. With prices being comparable, except the included insurance is much cheaper. My guess being that for people booking on sixt.com, insurance becomes a significant source of additional income, while for European customers, they still want competitive prices.
