The most likely thing is occasionally when you booked a flight using your UA account you missed changing to your other *A account. This may have caused you to earn UA lifetime miles.
A bit more random, there has been a report in the past that UA United Club agents replaced a non-UA frequent flyer account with the flyer's UA account upon entering the UC.

But no change in policy, to earn lifetime miles, your UA MP account needed to be on the PNR, and not the other *A account.
