LMU's are always offered on every AC flight as long as there is 1 J seat available. Being on the upgrade list does not effect anything and the LMU can be purchased up to 1 hr before departure.
It was not made available to me as an option via OLCI - I had to request the lounge agent to check the price for me (who somehow also unloaded me from the eupgrade list during the process).

2. No LMU offer at OLCI

I didnt know there was even emails out for LMU but since I have not received the supposed lmu offer email - the answer to your Q would be 3.
