The information provided is to ensure they find the right booking and that you should have access to it. The passport field is optional (and I personally wouldn't fill it in). I doubt you'd get anywhere with the ICO. You have a choice to not input any of said data and instead phone them if you'd prefer. On the phone they will most likely ask for a lot of the data that the online form asks for.
