Full Flex for a first time flyer ?

Hello,

Next year I will be flying for the very first time.
It will be with ANA from Brussels to Tokyo.
As I have some back pain (light herniated disc and sciatica) I was thinking about going for Premium Economy and an aisle seat to maximize comfort.

But my main question is about the differences between Premium Economy solutions.
I can choose between Value, Standard and Full Flex.
For a first time flyer I was thinking about going with Full Flex for "safety" reasons (if I got sick, etc... I can have the full refund).
But it adds 460€ per flight.

Value : 1662€
Standard : 1812€
Full Flex : 2582€

And if I take Value or Standard, it says there is only one seat left, which I don't see if I take Full Flex + it allows to upgrade to Business class.
I am OK to put extra money the reasons described above (if I got sick, or whatever the reason is), but I wanted to know if it's really necessary.
