5G at Sakura isn't enabled by default, there is an option in the account management to unlock it. I probably should also mentioned that I found the initial setup somewhat convoluted. Not sure if this is normal, but on iOS it required to download a file (APN settings) which came with an unsigned warning. They have a guidance video about that uploaded here:

I had a chance to try out Sakura Mobile a few weeks ago and it worked great with the voice number option. I also turned on 5G and that seems to help with the speed. When I landed in Tokyo my iPhone automatically notified me of pending eSIM to be installed, although I missed the instructions after I collected the instructions package requiring me to load onto their website to activate for good, but their support was prompt and got me on the right track.
Like you mentioned Sakura Mobile requires one to install an unsigned profile to configure APN setting, which does intefere with the other SIM/eSIM that you might have, such as my US T-Mobile free slow roaming or IIJmio backup, which is a minor annoyance. I only wished it could be like IIJmio where no profile is needed.
