THANKS, That strangely works (using Private Tab in Safari). Also, note that on the dates form, you may need to press tap on the keyboard to move onto the next field and make the entry stick. Several times, the entry turned blank on the DoB field, when moving onto the next field...hence being unable to use the 'Submit' button.
