I had the same issue and it transpired that the APIS information I had entered (including first and middle name as shown on my passport) did not match the details on my BA Executive Club account which only had my first name. After five or six complaints, they changed the details in my BA A/C to match the APIS detail and it’s been fine ever since. In terms of getting their attention, I made a complaint after the sixth call to get things put right manually.
