Indonesia not being on the list of approved passports kind of makes this a moot subject. You should probably make the reservation in the name as described above but only once a visa has been approved.... keeping in mind an appointment at the Embassy will be necessary and it may take time with no guarantee a visa will be issued, you should consider either making fully refundable reservations or waiting until the visa has been granted before making a reservation.
