They already know a ton about you. Full name, birthday, e-mail address, mailing address, etc. The vast majority will auto-match, all they have to do is clean up the stragglers.
That may be me! I remember a day going to my doctor’s office. At checkin my medical ID was shown, the appointment time was checked, the copay was higher than I was used to, but the desk person insisted it was fine, so I sat down and waited. Soon my name was called, and I couldn’t understand why the one in front of me in line accepted my appointment. It turned out we have similar names and the times were close to each other. I got refunded the difference in copay.
