I've successfully paid cash for 2 passengers and received in app offers for a previously flown SFO-DFW. I am unsure if 4 passengers in a single PNR will make it difficult to receive in app upgrade offers.

I regularly fly with four on one PNR and the in-app upgrade offers still come through. I even have a PNR with 5 on it, and there is an upgrade offer there as well. These are both paid and award (miles) tickets.
I have never actually upgraded a flight with four or five people. I wouldn't put it past AA's IT to have coded this incorrectly and for it to error out for one reason or another. That being said, just spot checking the offers I do have, it appears there are enough first class seats to accommodate four or five people when the upgrade offer is presented. If someone on here sends me the money, I would be happy to test AA's IT out.
