If you are trekking, the reputable companies won't take you without medical/evacuation insurance. A helicopter comes in at around US$5,000.
If not trekking and just visiting towns / cities, I am not aware of any requirement, and we were never asked to provide evidence of any coverage, but for peace of mind I'd have it in place.
How is the cost of evacuation relevant if you're required to insure for it anyway? Or, maybe you mean that the insurance policy itself is $5000?
