2) They should not require a new photo. They will be using the exact one you submitted for the pre-lim. If the photo doesn't "pass" their requirements, I imagine you won't be approved through pre-lim at all, rather than showing up and being asked to take a new one.
For whatever reason they didn't use my submitted photo and took a new photo right there at the Haneda site for secondary inspection. No issues.
