Well, it sounds like I was not the only one having this problem, and mine is not a family account! I also tried to call customer service to no avail. The only way I was able to eventually get around this problem was to change the phone number in my account using my Italian phone number instead and then, the OTP came through immediately! Something to do with US phone numbers? Quite weird.
I was then the able to take advantage of the 5% cash back.
