Helpful, we plan to hit a couple of these while in Sicily, and it has been awkward for me in other parts of the world (in Peru and Portugal certainly got the feeling it was expected). and even in Rome many moons ago when we ate at La Pergola -- tip or not, how much if so. In the case of a Michelin star spot, if we don't have to tip like an American, would leaving 5 Euros be appropriate if the service was exceptional or would that be insulting?

Tipping question, if we pay by credit card should we expect to see a tip line (sounds like no) so assume just keep small bills and coins handy.

And truly random, if we plan to grab a taxi at the airport how best to say we expect/want the flat rate? And I assume cash, so is there a decent ATM in the airport before we exit (I don't think I have any Euros in my passport)?
