Flew the 764, 339, and 359 in Delta One in the last month several times, and other than th short transcons, I had back pains in every one of those long hauls. I think the seats are just too flat and hard and lacks contouring. I flew Latam 763 and older 2-2-2 Dreamliners many times too, and bulkheads in their 763 and newly refurbished 789s are fine. Only once did I wake up with a back pain in Latam's 2-2-2 seat. On the way back from JNB to JFK, I opted for KLM and did not have back pains whereas the way down via ATL on Delta's 359, it was bad. I actually told Delta's FA on my recent Buenos Aires trip on 330neo (both times in bulkhead window) that AA's seats are a lot more comfortable
I really, really liked business class on the KLM 787-10 (we flew one on its third or fourth voyage ever... then Covid..)
Great cabin, great seats.
