Engines for one, RR (YMMA upwards) on some GE on others. For cabin crew different location of the bunks (far better on the 300's). In my time some of the 200's had the flight crew bunks at the back of First in the cabin (on the K side), off the top of my head I think it took the space of 2 seats.
Here's a picture of the outside of the rest area, taken on the way back from Tokyo in 2016. There were two seats missing but the rest area didn't take up all the space... it looked very much an afterthought and I can't imagine it was very comfortable inside!
