While in the lounge waiting for the connecting flight my wife's knee popped and we had to return home. I don't think you situation really had that much to do with the companion certificate. You changed your itinerary. These certs have inventory restrictions among other considerations. It wasn't Delta's fault that you had a medical situation. Delta did bend the rules to try and accommodate you. In the future, you might consider buying insurance.
