It depends which FF program you book the JL award through.

Some (e.g. Cathay) will tell you to book the adult seat with them, then deal directly with JL to attach the infant to the booking. JL will charge 10% of the adult fare.
Some (e.g. Qantas in my case) allow you to book the infant as part of the same PNR. They apply their own infant policy.

Qantas doesn't charge any additional points for infants - only (real) taxes.
