In the USA it is hard to think of a well-designed intl-dom transfer. BOS, JFK, IAD, DFW, LAX, SFO, etc. are all so time-consuming and serpentine, with tons of walking or shuttles or going outside. Perhaps DTW and MSP, as compact one-roof experiences, edge LHR but few others. ORD is awful now but will be good when the new Global Terminal comes online in about ten years.
A more limited option, but one that worked for me when I had work trips to Sacramento was SLC. The route is well designed, the process was slick, the gates staffed properly, the people, even CBP, were polite and friendly and they gave the impression they actually liked the idea of passengers.
