This page is used to claim missing flight credits, but there is a glitch in the programming: after you enter the ticket number for the missing flight, the next screen prompts for all the flight data. But it's impossible to enter the flight date.
The flight date prompt shows a calendar, but the underlying link goes back to:

Restarting the entire process.
I have attempted to use the page on multiple platforms, but the results are the same, no joy.
No idea who at United I can contact to get this fixed.
I was ultimately able to get flight credit, but then I flew TG BKK-HAN. When I checked in, they told me they couldn't apply my UA # into the system, they received an error message "No service". Their opinion was that my name didn't match, which is odd, since I have flown TG dozens of times using my UA MP number.
Anyway, I will have to contact United again to get credit for my flight.
Meanwhile the website glitch for Missing Flight Credit has not been fixed.
