On Transatlantic upgrades: I've been doing 1-3 trips a year "forever" with a 90% success ration. Buy Y, B, M tickets. pay 40k miles, book a month ahead, fly mid-week and take the 1st flight of the day to LGW and CDG and similar. Sometimes it helps to fly to more obscure places like DUB or AMS.
