Every resort uses bottled water to cook with. Just avoid anything with fresh vegetables. That's for any country. The same thing happens in the USA on a daily basis.
The other thing is ice (offsite). I assume the entire resort water systems (including what they clean vegetables with) is from the same purification system - whether the shower, the kitchen, etc. So any fresh vegetable ailments are from the field, or handling, same as any fresh Mexican fruit/veg imported into the USA.
And finally, shellfish toxins of the type you can't cook out are troublesome. Good thing I don't like oysters, because on one Cabo trip, there were five of us out to dinner. One started getting crampy on the walk back to the hotel, and within 24 hours, the other three were suffering various forms of exorcisms. I was fine. And the oysters were cooked.
I also expect that a lot of gringo gastrointestinal ailments are caused by too much tequila and too much spicy food, but blamed on cleanliness.
