I think the cutoff is 20h, and this was the shortest connection I was offered, from Boston to Dublin ultimately. You get one per itinerary according to the published rules. There was absolutely no quibbling.
I was surprised by the limo too - I wasn't even expecting a transfer. The dude asked if I was elite or elite plus and when I showed my card he said it was for a nicer transfer. I was also surprised to get a return transfer the next day, same limo, which is a little black van with blackout curtains, a sort of star field of LEDs on the ceiling, and a large widescreen TV at the front. Possibly it's a bit of front end schmoozing for a new Elite member. I just assumed it was standard.
