I have the same rules (well, to be fair, I didn't check for the extension after the first two years), and I've did the flight within four months (actually within about two weeks) and the 15,000 status miles within 12 months but the date on the website/app shows doesn't show the extension to two years. And the helpdesk person just...not helpful. So, I'm assuming/hoping that it will show the 2025 end date when the 2024 end date comes next month.
I'll have reached over 37,500 from the start of the status match by this November but assume it will not show the further extension (assuming I have the same rules - I'll check) until October 2025. Weird system.

Weird system indeed.
To make it even weirder, after the 2 years my status was decreased despite having the required 37.500 miles. 3 feedbacks sent and every time the same answer that I did not meet the requirements for further qualification. The system is probably not programmed for statusmatches.
So I searched for all the previous feedbacks of those 2 years and found the one in which they stated that those 37.500 miles need to be earned from the start date of the statusmatch. That probably did the trick and they sent me a reply with apologies and reinstated my Elite status. (in the mean time 4 flights statusless, no lounge, no priority security and pay for hold luggage).
So please be prepared.
