The upgrade is only for the specific flight you have requested it for. If you change the dates, you will be back in economy class for the new itinerary - the upgrade does not transfer across to a different flight.
I also don't believe you get it back if you cancel the flight or change the flight either - but that I am not entirely 100% sure about that. My impression is, you ask for an upgrade, it's taken from your account if it's confirmed, and that's that. If you cancel or change the flight, it's not returned to your account for later use.
