The GoC website still tells you which stores theoretically sell them. Can be hit or miss, since stores run out of them.

Farewell sweet prince. Was fun while it lasted. No idea what triggered it, as I hadn't been doing any big volume recently. I also got past a temporary suspension a couple years ago when my account got frozen and I had to have a call with a rep from TIAA (the admin of my state's plan) but was cleared after that.
Were you regularly withdrawing from the account, or just doing big withdrawals every so often?
