Regarding flexibility, you are right. If the upgrade request goes through, flexibility is dead.
We generally only request the upgrade when we are sure we are going to fly, typically a week out. (using the 7 day pass).
Even without the upgrade going through, you have to call in to ask to remove the upgrade request so you can change the flight.
