Baggage allowance and medical devices

I’ll be travelling on AA metal first TALT, then BA metal UK and then TALT. I know that my C-PAP bag is excluded from baggage constraints both in the U.S. and TATL, on a U.S. carrier, by FAA rule. It is sized to only hold the C-PAP, no more or no less. How will this be treated on both BA and CityFlyer internal flights and BA TALT. The size, if relevant, is roughly 35x26x16cm, and I have a letter of medical necessity. Will this displace my personal item on these flights, as I would prefer to use a compliant roller bag and a small backpack (personal item) for HBO purposes…but I’m unsure how this C-PAP bag would be treated. (Previous trips were conducted with the roller only plus the C-PAP for shorter trips and checked baggage for longer…this trip falls slightly longer than the roller alone is ideal for)

If it’s “unclear” as to the treatment, my intention is to pack the essentials in the backpack and check to roller bag, but that’s a fallback position. I know this is the BA forum, but any other OneWorld treatment data would also be appreciated (IB and JL)
