Yes. BA require the 12 year old to be travelling
with someone 16 or over, i.e. on the same itinerary. They don't need to be on the same booking, but it will require a phone call.

Since you are in a HHA, as you no doubt know, you can only book for those in your account, or on your Family & Friends list. As a 12 yo, he can only have a BAEC account by virtue of being in a HHA, so he would ether need to join one created by his parents, or yours as far as I can tell

RFS operates off the person spending the Avios to my knowledge, not the traveler. Since you are redeeming, RFS should be available.

A good question, formally they should charge the fee but in practice they will often waive it. The fee isn't waved for things that
can't be done on the website is as sometimes thought, it's waived for things that
should normally be possible on the website but cannot be completed due to issues with the website. I would ask them to do so anyway, see what they say.
