Don't need to call and wait on hold. PQP should be broken out on your reservation email if purchased directly from United.

Manage trips on the UA website (desktop) should have it; though I am not 100% certain.

There is a manual calculation you can perform by finding out relative % of each segment to overall distance, and multiply by the overall PQP.

And yes, crediting is based on the scheduled departure date of the flight. 2024 flight would credit to 2024, 2025 flight would credit to 2025.
