My two cents:

I have Blue Cross / Blue Shield (high plan) through my employer as well as Tricare (retired military). When I went to Thailand earlier this year I suffered a broken foot. I went to Samitivej Hospital in Bangkok for treatment. I paid upfront and then submitted a claim with BCBS and Tricare. Both ended up reimbursing the entire bill. ​​​​​​This is the normal coverage I get at home. However, the insurance company gets direct billed from the provider.
