Medical travel insurance for US citizen traveling to Asia. Costs / best companies?

I'll be traveling to Asia (Thailand and Japan) this week and wanted to know what a good medical travel insurance policy would be and approximate costs. I have Chase Sapphire Preferred card if that helps (in case there are any discounted plans through it). Preferably would include Medivac coverage / flights home if required. I have a Blue Cross Blue Shield plan at home and I'm not really sure what it covers. My trip duration is 3 weeks.

I'm looking at InsureMyTrip.com and it shows a number of providers. I don't know which are good or bad, and it's probably a mistake to just choose the cheapest plan.

I'm also confused because, while I don't really need "comprehensive" insurance (I leave in a few days, and I believe my Chase Sapphire Preferred has free travel interruption insurance), the comprehensive plans seem to be cheaper than medical evacuation plans, even though there's verbiage saying "comprehensive" often covers med-evac.
