Sushi Take meets all your requirements, except it might be just a tad over the Y20,000 for the full course with otsumami. I think last time I paid Y23,000 - but that was with a couple of drinks.

It's my other regular spot along wih Taichi (where I've been going for over ten years and have been to probably close to two dozens times.) She trained for many years at Shimbashi Shimizu. Really excellent spot, highly recommended.
