Ok, based on Jetblue website:
Departures
5:00am - 8:30pm: Terminal 5
8:30pm - 5:00am: Terminal 4

Ironically my flight may be at 8:30pm, so is it a crapshoot? Actually , looking at their flight tracker, seems all flights today even late ones (though no 8:30) are from T5 like you said

Might fly from LGA, I think Jetblue is Terminal B and so is the Sapphire lounge . I dont think they have other lounges
