I have had similar issues domestically and internationally. While I am sure all of us expect 100% quality control in the cleaning process, the fact of the matter is that something is going to slip through and make it to your tray. This is compounded by the fact that the FAs do not see or check the utensils prior to distribution.
Now dont get me started about a FA who gave me a dirty look when asked to replace a nasty knife I got on a flight to HNL last year....
